Description
MEXAFIX-O contains Cefixime 50 mg and Ofloxacin 50 mg in a Dry Syrup formulation. Cefixime is a third-generation cephalosporin antibiotic, while Ofloxacin belongs to the fluoroquinolone class. Both medicines act against susceptible bacteria through different antibacterial mechanisms.
MEXAFIX-O is supplied in a 30 mL pack. Because the combination contains systemic Ofloxacin, its use requires careful clinical assessment, particularly in children and adolescents. Ofloxacin carries important safety warnings, and official systemic labeling has not established safety and effectiveness in patients below 18 years.

Uses of MEXAFIX-O
MEXAFIX-O may be considered by a qualified healthcare professional for selected bacterial infections when the causative organisms are susceptible to both components.
Your supplied product information lists:
- Pediatric bacterial infections
- Respiratory infections
- Urinary Tract Infections
- Diarrhoea associated with susceptible bacterial infections
- Typhoid fever
However, these claims should not be interpreted as universal indications for every cefixime/ofloxacin product. Cefixime labeling supports selected infections such as uncomplicated urinary tract infections, otitis media, pharyngitis/tonsillitis and certain respiratory infections.
In addition, official Indian CDSCO records list certain Cefixime + Ofloxacin fixed-dose combinations for typhoid fever and urinary tract infection in adults. That does not establish routine pediatric use of the combination.
Key Benefits of MEXAFIX-O
- Combines Cefixime 50 mg and Ofloxacin 50 mg
- Contains two antibiotics with different antibacterial mechanisms
- Available as an oral dry syrup
- Convenient 30 mL packaging
- May provide broad antibacterial coverage against selected susceptible organisms
- Can be considered only when the combination is clinically justified and appropriate for the patient’s age and condition
Because unnecessary use of multiple antibiotics can increase adverse effects and antimicrobial resistance, combination therapy should follow appropriate professional assessment.
How MEXAFIX-O Works
MEXAFIX-O contains two antibacterial agents.
First, Cefixime interferes with bacterial cell-wall synthesis. It binds to bacterial proteins involved in cell-wall formation, weakening susceptible bacteria and preventing them from maintaining their normal structure.
Meanwhile, Ofloxacin belongs to the fluoroquinolone class. It interferes with bacterial DNA replication by inhibiting bacterial enzymes involved in DNA processing.
Therefore, the two components act through different antibacterial mechanisms. However, the combination does not treat viral infections such as the common cold or influenza.
Common Side Effects of MEXAFIX-O
Possible adverse effects may include:
- Diarrhoea
- Nausea
- Vomiting
- Abdominal discomfort
- Indigestion
- Headache
- Dizziness
- Skin rash
Ofloxacin can also cause clinically important fluoroquinolone-associated adverse reactions, including tendinitis and tendon rupture, peripheral neuropathy, central nervous system effects and worsening of myasthenia gravis.
Seek medical attention promptly if severe diarrhoea, severe rash, breathing difficulty, tendon pain or swelling, numbness or tingling, unusual weakness or significant neurological symptoms occur.
Important Safety Information
- MEXAFIX-O should not be presented as a routine pediatric antibiotic. Systemic Ofloxacin labeling states that safety and effectiveness in patients below 18 years have not been established.
- Use this combination only when specifically prescribed by a qualified healthcare professional.
- Do not use antibiotics for viral infections.
- Patients with previous serious reactions to cephalosporins, fluoroquinolones or other relevant antibiotics should inform their doctor.
- Ofloxacin carries important warnings concerning tendon injury, peripheral neuropathy, CNS effects and myasthenia gravis.
- Patients should report tendon pain, swelling, numbness, burning, tingling or weakness promptly.
- Patients with kidney impairment may require appropriate dose adjustment.
- Avoid unnecessary antibiotic use because inappropriate exposure can contribute to antimicrobial resistance.
- Severe or persistent diarrhoea during or after antibiotic treatment requires medical assessment.
- Do not share this medicine with another person.
- Keep the medicine out of reach of children.
For cefixime, official labeling recommends using the drug only for infections that are proven or strongly suspected to involve susceptible bacteria.
Dosage & Administration
Use MEXAFIX-O only according to the prescription and product-specific instructions provided by a qualified healthcare professional.
The appropriate regimen depends on:
- Age
- Body weight
- Infection being treated
- Severity of infection
- Kidney function
- Bacterial susceptibility
- Other medicines being used
- Individual risk factors
Do not calculate the dose from the 30 mL bottle size.
For cefixime alone, official labeling uses weight-based pediatric dosing in children six months and older. However, those cefixime dosing recommendations cannot be automatically applied to this Cefixime + Ofloxacin combination, particularly because systemic Ofloxacin has important pediatric restrictions.
For the dry syrup, follow the manufacturer’s exact reconstitution instructions. Add only the specified quantity of water, shake thoroughly and administer using an appropriate measuring device.
Do not estimate the amount of water required for reconstitution.
Storage
- Store the dry powder according to the manufacturer’s instructions.
- Keep the bottle tightly closed.
- After reconstitution, follow the specific manufacturer’s storage conditions and discard period.
- Shake the suspension well before each use.
- Keep out of reach of children.
